Duties:
- Lead the research and implementation of best practice guidelines for assessing individuals with ID/DD seeking long-term care.
- Provide timely technical assistance to assessors and evaluators, fostering informed decision-making and community integration.
- Develop and execute comprehensive tracking strategies to document services and monitor outcomes effectively.
- Collaborate with the Home and Community Based Services division to pioneer specialized support services for individuals with ID/DD.
- Bachelor's degree in behavioral health or related field.
- Minimum 1 year of hands-on experience supporting individuals with intellectual disabilities/developmental disabilities or related conditions.
- In-depth familiarity with mental health support systems and community resources.
- Proficiency in Excel and exceptional communication skills.
- Knowledge of data tracking methods and confidentiality protocols.
